For the most part, our students give out questionnaires to other students. In such cases, they should inform participants orally and in written form. We suggest a cover page to survey, with above cited information. They can tear the off the cover page and keep for their information. For example, the cover page could read:
My name is ________, I am conducting a study on romantic relationships for my adolescent psychology class. I am asking you to participate by completing the survey. The survey asks questions pertaining your behaviors and feelings in romantic relationships and
mood. Your participation is voluntary which means that you do not need to participate if you do not want to. You will not be penalized in any way for not participating. Likewise, you do not need to answer any question that makes you uncomfortable and you
may withdraw for this study at any time. Your participation is anonymous so DO NOT PUT YOUR NAME on this questionnaire. There are no known risks to completing these questions. If you have any question, please feel free to ask me. If you have any question regarding your rights as a participant, please contact Dr. Chairperson at 447-3297. If you would like a copy of the results of this study, you may contact Dr. Advisor.
